Theo – Interview with motherMade-up example · Developmental history · Date in the file name: 14 September 2026 · 52:40 · 2 speakers
TranscriptNoteIn their wordsSpeakersTo verify

Interview with Speaker 2, who says she is Theo's mother, conducted by Speaker 1 at the start of an assessment the school suggested.

Reason for referral

According to Speaker 2, the teacher reported that Theo loses focus, rarely finishes written work and copies slowly from the board. Her main worry is reading: he still reads "sounding out every word".

Pregnancy and birth

  • Planned pregnancy; gestational diabetes from "about the sixth month", managed with diet.
  • Caesarean at 38 weeks; birth weight "about seven pounds, I think".
  • Mild jaundice, one day under the lights; home after two days.

Development

  • Sat at about six months; walked at 14 months.
  • First words "a bit after one"; short sentences around two and a half; speech therapy from four, for about a year.
  • Dry by day at three, at night around four and a half.
  • Takes over an hour to fall asleep; comes to the parents' bed two or three nights a week.

Health

Repeated ear infections between two and three. A hearing test at about five: "I think it was normal". No regular medication.

School and learning

  • Third grade. Answers well out loud but "freezes" when he has to write, as Speaker 2 relays the teacher's reports.
  • Homework meant to take 15 minutes takes an hour.

Family and social life

Lives with both parents and a sister, 3. Two close friends; football twice a week.

Behaviour and emotions

  • Cries when his team loses and sometimes leaves the game.
  • Says "I'm stupid, I can't do it" when he gets homework wrong.

Points to verify

  • Hearing test: name and result to confirm; Speaker 2 will look for the report.
  • Birth weight given from memory.

A whole assessment in one go

Drop in every recording for a client at once: OBS videos, phone audio, interviews with parents. Each one is transcribed with the speakers separated, and files you have already added are flagged.

You say who is who

The transcript shows Speaker 1, Speaker 2 until you name them or pick a role: clinician, client, mother, father, teacher, co-therapist. Nothing guesses who the clinician is.

A draft note for every recording

A session note, SOAP or DAP note, intake assessment, developmental history, feedback, supervision or crisis-session note, chosen from the file name and easy to switch. Only what was said: unclear passages are marked, and anything said about risk is quoted word for word, never rated.

The client file across sessions

Every session in the order of the date in its file name, with its plan or agreed next steps; the open points from every note; and a list of sessions with durations and speakers.

Word, PDF and plain text

Export the transcript with timestamps and speaker names, or the note, and finish it in your own documents.

Private to your account

Recordings and transcripts stay on your txscribe account, are not used to train models, and you can delete them at any time.

How it works

  1. Create a client (initials or a code are enough) and drop in the session recordings.
  2. Name the speakers or pick their roles; each recording gets a transcript and a draft note.
  3. Review and edit the drafts, then export them to Word or PDF.

Questions

What files can I upload, and how long can they be?

Audio and video files from your computer or phone: OBS or video-call recordings (.mkv, .mp4), voice memos (.m4a, .mp3), WAV and other common formats; not links. Long recordings are fine, several hours each: full sessions, testing sessions and parent interviews go in as they are.

Which notes can it draft?

A session note, a SOAP or DAP note, an intake (first-session) assessment, a developmental history, a feedback-session note, a supervision note or a crisis-session note. The kind is chosen from the file name ("devolutiva", "primera consulta", "supervisão", "SOAP"…) and you can switch it. In SOAP and DAP notes, Assessment holds only what you said aloud in the session; otherwise it is left for you to complete. Intake and crisis notes quote what was said about risk word for word and never rate or classify risk: they are drafts and do not replace your own risk assessment.

Does it know who is the clinician and who is the client?

No, and it does not guess. Speakers appear as Speaker 1, Speaker 2 until you name them or pick a role (clinician, client, parent or guardian, mother, father, teacher, co-therapist). The notes use the names the transcript has.

Do I need the client's consent?

Yes. Recording a session needs the client's consent, or a parent's or guardian's, under the rules of your profession and your country. This app only works with recordings you already have.

Where do my recordings go?

To your txscribe account. They are not used to train models, and you can delete any recording, transcript or note at any time.

What does it cost?

It uses the minutes on your txscribe account, like any file you transcribe there. You start with free minutes.

What is it not?

It is not a dictation app and not a records system. It transcribes sessions you recorded and drafts notes from what was said; it does not diagnose, interpret or assess risk, and a draft is not a clinical record. Review and edit every draft before filing it.
